Forms
LAP is always looking to expand our services. If instructors desire a Supplemental Instructor, they can request one with this form. After completing the form, please return it to studentsuccess@nsu.edu.
Faculty and Staff can also request mentors for university activities. Completed forms can be returned via email to studentsuccess@nsu.edu.
Want to be a mentor?
Students must have:
- A 3.0 GPA
- A letter of recommendation from a faculty member
If you are interested in becoming a mentor, please complete the application. Please return completed applications to studentsuccess@nsu.edu.
Want to be a tutor?
Students must have:
- A 3.0 GPA or higher in courses which they want to tutor
- A letter of recommendation from a faculty member who teaches in the subject area for which they want to tutor.
